




Fees and Policies
Payment Policy
Payment is due at the time of the consultation. Although we do not file insurance claims, we will gladly provide you with an itemized claim for each visit. The claim can be submitted to your health insurance carrier and you may receive reimbursement depending on your out-of-network benefits.
Cancellation Policy
All appointments are confirmed by our office at least one week prior.
Twenty-four hour notice is required for all cancellations. Appointments cancelled within the twenty-four hour period or “no shows” are subject to a fee. Please call our office if you are concerned that you will be late. We will do our best to keep your appointment, but we must know in advance for the consideration of our other patients.
Medical Coverage
All patients are strongly encouraged to maintain an ongoing relationship with their primary care physician and any indicated specialists. Dr. Elmaleh does not provide emergency services. She focuses on preventative healthcare and the treatment of chronic medical conditions. Please see the full list of conditions treated here, and call our office if you have a concern about a condition not listed. Patients with questions may call during regular business hours or email our office.
